Parents must bring physical forms with them to the appointment. Parents must fill out their portion of the paperwork prior to arrival. If incomplete, this may result in loss of scheduled appointment time. Lyster does not have copies of local school forms.

Only school/sports physicals will be addressed. If there are other concerns or issues (medication refills, referrals, acute issues, etc), please make an appointment during regular clinic hours. If the child wears glasses, please bring glasses to appointment for eye exam portion of physical.

If experiencing any COVID like symptoms, please reschedule appointment for when healthy. Patient must be present for appointment. Parent cannot bring form to appointment without child present regardless of how recently a child has been evaluated by their PCM. Do not bring additional children to the appointment. If additional child(ren) require a sports physical, each child requires their own appointment.
