Allergy/Immunology Service
Immunization visits are on a walk-in basis; no appointment is needed.
Check in at the Primary Care Clinic Kiosks and click the "Walk-In" option on the screen. The front desk MSA will call your ticket number, and you can proceed to the front desk to complete your check in process.
A valid military ID card is required for all visits. There are no exceptions.
The influenza vaccination is administered on a seasonal basis. Please contact our clinic for more details.
Active Duty Soldiers scheduled to PCS to OCONUS assignments and those deploying overseas require a DA 4036-R from their unit. Vaccinations cannot be given without this form.
Alabama Blue Cards are issued in this clinic. A box is located near the check-in kiosks where you can fill out a form and drop in the box. The immunization nurse will prepare the blue cars and call the patients within 72 business hours to pick up the card. When returning for pick up, please pull a ticket at the kiosk first. The MSA will call your ticket number when it's your turn at the desk to pick up your completed blue card.
Immunization Clinic is only available to patients with a Lyster PCM.
