All adults are required to possess valid ID. All juveniles without valid ID must be accompanied by an adult family member. Drivers must be able to show:

  • Valid driver's license

  • Proof of current insurance
Due to COVID-19 pandemic new visitation guidelines are necessary to ensure we prioritize patient and staff safety.
We remind everyone of the importance of Reporting All Symptoms prior to entering the clinic.
Visitor Screening: During the screening process prior to entering the clinic, staff will ensure you have a mask on and then ask if you are experiencing any COVID-like symptom(s) or if you have been exposed to someone positive. It is your duty to report any symptom or exposure at the time of screening.
Even if you think it’s just seasonal allergies, or a child’s ear infection, you must report it. A nurse or medic will evaluate the symptom and decide where your appointment will take place. If you fail to disclose COVID-like symptoms or exposure at the time of initial screening, you could lose access to care at Lyster.
We are in a fight to stop the spread of COVID; we all play a role in ending this pandemic.
COVID-like symptoms include: Fever or Chills, Cough, Shortness of Breath or Difficulty Breathing, Fatigue, Muscle or Body Aches, Headache, New Loss of Taste or Smell, Sore Throat, Congestion or Runny Nose, Nausea or Vomiting, Diarrhea.
If you are experiencing a COVID like symptom, we encourage you to call the clinic prior to coming to an already scheduled appointment (such as well baby, routine health appointment, etc) for guidance on how to proceed.
Patients may have one visitor if they need physical assistance for an appointment or support for cognitive or behavioral impairments.
