Location: Immunization Clinic, Primary Care Clinic (PCC)

Current Requirements

Due to COVID-19 pandemic new visitation guidelines are necessary to ensure we prioritize patient and staff safety.

Face masks are required. Please ensure your mask is on during your visit to the clinic.

Effective, 25 July: Self-screen prior to entering the LAHC.                      

  • If you are not experiencing any COVID like symptoms, you may proceed directly to your destination.
  • If you are experiencing any COVID like symptoms listed below, do not enter the clinic. Call the Lyster AOD Desk at (334) 255-7700 for assistance
COVID-like symptoms include: Fever or Chills, Cough, Shortness of Breath or Difficulty Breathing, Fatigue, Muscle or Body Aches, Headache, New Loss of Taste or Smell, Sore Throat, Congestion or Runny Nose, Nausea or Vomiting, Diarrhea.

We are in a fight to stop the spread of COVID; we all play a role in ending this pandemic.
 
Patients may have one visitor if they need physical assistance for an appointment or support for cognitive or behavioral impairments.
